Tips for Effective Loss Reporting

Please fill in the loss notice form as completely as possible.
Fill out a separate loss notice form for each insured. For example: If a doctor who is a member of a PA is reporting an incident or loss, fill out one form for the doctor and one form for the PA.
When describing the loss, please be as brief and concise as possible. Only ask for a basic description of the loss. The insured will be discussing the claim in detail with the assigned claim representative as well as an attorney in some cases. We want to avoid leaving the insured with the feeling that he / she has repeated the same information excessively.
It is important to report incidents that may lead to a claim as well as actual claims and suits. It is to the insureds advantage that the SCJUA know about any possible claim as soon as possible.
Whenever possible, please fax the loss notice to the SCJUA. Timely reporting results in a more efficient claims process.
Please provide any additional relevant information when faxing the loss notice including phone number, fax number, and e-mail address.
